54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted from 9:00am -11:15am. Upon arrival there were 5 children, totaling 11 points with 1 caregiver. All 5 children's records were reviewed in addition to 4 caregiver/household records. The children were observed eating oranges for a snack, during a potty break and playing in centers.
Standard #: 22VAC40-111-60-B Description: Based on a record review the licensee did not ensure all required information was kept for each child Evidence: The licensee is also a provided for the Navy. The licensee used the enrollment form provided by the Navy, which did not include the following requirements for a licensed family day home: 1. Emergency contacts to include name, phone number, full address 2. nickname 3. Chronic physical problems, pertinent developmental information, and any special accommodations needed 4. Allergies 5. Custodial parents rights policy 6. communicable disease policy Plan of Correction: The following corrective action was provided by the licensee during the exit interview: The licensee will have parents fill out the model from provided by Social Services in addition to the Navy form to ensure all required information is captured.
Standard #: 22VAC40-111-70-A Description: Based on a record review the licensee did not ensure all required information was provided to the parents before the child's first day of attendance. Evidence: The "Information For Parents" form that was in the children's records did not include the following required information: fees, check in and check out procedures, daily schedule, termination policy. Plan of Correction: The following corrective action was provided by the licensee during the exit interview: The Information For Parents will be completely filled out and given back to parents for signatures. a copy of the signed forms will be kept in each child's folder.
Standard #: 22VAC40-111-240-A Description: Based on observation the licensee did not ensure all areas and furnishings of the home were maintained in a safe condition. Evidence: In the living room, children were observed watching television. The large screen television was on a coffee table stand and was not secured to prevent tipping Plan of Correction: The following corrective action was provided by the licensee during the exit interview: The licensee added a support at the base of the TV but will have it mounted to the wall to ensure safety.
