Comments:
An unannounced monitoring inspection was conducted on September 27, 2018. At the time of entrance there was 1 provider and 9 children with a total of 17 points. The sample size consisted of 5 children's records and 1 provider record. Children were observed watching television, preparing for nap and during their nap time. The provider does not administer medications. The outdoor area was not inspected due to inclement weather. The inspection began at approximately 12:15 pm and ended at 2:00 pm. The results of the inspection were reviewed with the provider during the exit interview. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review and interviews, the provider did not ensure to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: During the inspection on September 27, 2018, the record for child #2 first date of attendance was 7-5-2018 did not include a physical from a physican.

Plan of Correction: The provider will have parent get doctor to complete physical.

Standard #: 22VAC40-111-140-B-3
Description: Based on record review and interviews, the provider did not ensure a caregiver had obtained current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department. Evidence: During the inspection on September 27, 2018, the most recent CPR card was dated 4/2016 and it indicates expiration is 4/2018.

Plan of Correction: The provider will make appointment to take course.

Standard #: 22VAC40-111-180-A
Description: Based on record review and interviews, the provider did not ensure to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements ,every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: During the inspection on September 27, 2018, the most recent TB screening available for review for the provider was dated 6-24-2015.

Plan of Correction: The provider will get copy from physician of TB.

Standard #: 22VAC40-111-530-D
Description: Based on observation and interviews, the provider did not ensure linens were clean and washed at least weekly or when soiled. Evidence: During the inspection on September 27, 2018, the napping blankets were observed and they were stacked on top of one another. The blankets are considered no longer clean as they are in contact with one another. The provider confirmed the blankets are only washed on Fridays unless a child has a cold or something.

Plan of Correction: The provider will separate the items.

Standard #: 22VAC40-111-570-A
Description: Based on record review, observation and interviews, the provider did not ensure a caregiver does not exceed 16 points by using the point system to determine if an additional caregiver is needed. Evidence: During the inspection on September 27, 2018, 9 children were observed in care. The number of points totalled 17. There were 8 children assigned with 2 points each and one child assigned with 1 point.

Plan of Correction: The provider will have a child not come until another child turns 5.
