An unannounced inspection was conducted from 10:30- 11:50. There were 12 children (infants-4years) present with 2 caregivers for a total of 32 points. Some of the children were taken on a walk and returned during the visit. The Inspector could not check the outside play area due to the rain earlier. Activities included art projects, play dough, toys, and free play. Hand washing was also observed. Children's files, staff/household files, and attendance records were checked. Provider is not MAT certified and parents have signed that they understand she will not administer medication. Please call with questions 703-479-4679.

Description: Based on observation, the Provider cannot provide an individual crib for each infant. Evidence: 1. Four infants were present and the Provider only has two cribs available for use.

Plan of Correction: I will purchase two cribs.

Standard #: 22VAC40-111-540-A
Description: Based on observation, the Provider did not use an infant carrier according to instructions. Evidence: 1. Infant was downstairs in a carrier seat without a caregiver present. 2. Carrier's instructions state " Do not leave a child unattended"

Plan of Correction: I will make sure infants are in sight. I only left to bring older kids upstairs for an activity.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information.
