The Sworn Statement form was revised in May, 2017 to include the identification of residency outside of Virginia in the past 5 years.
An unannounced inspection was conducted in the Family Day Home on September 20, 2018. Upon arrival in the FDH at approximately 12:40PM, the LI observed the Provider and a household member present. There were two children in care. After school, a child was transported to the FDH by the school bus for afternoon care. The ages of the children ranged from 2 to 6 years, the points totaled five. Reportedly, five children are currently enrolled. The children were observed during nap, free play, hand washing, afternoon snack and dismissal. The LI reviewed five children's records. Also reviewed were records of the caregivers and a household member, documentation of practiced emergency drills, posted emergency numbers, and the first aid kit. As reported, the FDH does not administer medications. The findings were reviewed at exit at appromiately 3:30pm. If you have any questions, please call (540)309-2310. Thank you.
Standard #: 22VAC40-111-780-B Description: Based on documentation reviewed in children's records, the Provider failed to obtain the parent's signed acknowledgment of an annual review to ensure information is correct. One of five records had not been updated at least annually. Evidence: Documentation signed by the parent was not available for review for child 2 to verify that an annual review of the emergency contact information was conducted to ensure the information is correct. Plan of Correction: The Provider will review the information with the child's parent and will request that the parent complete an updated registration form for the child.
Standard #: 22VAC40-111-100-A Description: Based on a review of children's records, the Provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: One of five records reviewed did not contain documentation of a completed physical examination. Documentation of a physical examination for child #1 was not available for review, the child has been enrolled in the Family Day Home over 90 days. Plan of Correction: The Provider will inform the responsible person and will request a copy of the current physical, the document will be filed in the child's record.
Standard #: 22VAC40-111-60-B Description: Based on review of a sample of 5 children's records, the Provider failed to obtain a copy of documentation to verify legal custody of an enrolled child. Evidence: Documentation was not available for review to verify custody of child 1 whom was enrolled by someone other than the biological parent. Plan of Correction: The Provider will request documentation from the child's guardian. Documentation will be maintained in the child's record.
Standard #: 22VAC40-111-180-A Description: Based on reviewed documentation, the Provider did not obtain documentation of a current report of tuberculosis screening, every two years from the date of the first screening, in one of four reviewed records. Evidence: The available documentation of the TB screening for Assistant 2 was not current, the screening was dated 02/29/2016. Plan of Correction: The Provider will obtain documentation of a current TB screening from Assistant 2.
