54.1-3408 Provider must be MAT certified to administer prescription medication.
An announced initial inspection was conducted with the provider beginning at 10:15 AM and ended at 1:21 PM. There were three children in the care of three caregivers. The children were 12 months, 2, and 3 years old for total of 8 points. The children were observed playing with toys. Children were observed eating lunch which consisted of turkey slices, Hawaiian rolls, cheese, green beans, sliced strawberries and milk. One child was observed sleeping in a crib. Two children were observed sleeping on cots. Three children's records were reviewed. Two household members' records and three staff records were reviewed and in compliance. There were no medications on site. The provider has current First Aid and CPR certification. The provider also has Medication Administration Training certification. The The first aid kit, flashlight, and weather-band radio were observed and in compliance. Not all of the areas of the standards were able to be observed during the inspection. Standard #: 22VAC40-111-60-B Description: Based on a review of documentation, not all of the children's records were complete. Evidence: Child A and B were missing the phone number and address for parent's place of employment, the address for one emergency contact, and second emergency contact. Child C was missing the address for parent's place of employment and two emergency contacts. Plan of Correction: The provider stated that she would obtain the missing information from the parents.
Standard #: 22VAC40-111-520-C Description: Based on observation, not all of the cribs met the U.S. Consumer Product Safety Commission Standards(16 CFR parts 1219, 1220, and 1500). Evidence: One of the two cribs in the home was manufactured February 15, 1996. Plan of Correction: The provider stated that she would get rid of the crib and replace it with one that was in compliance.
Standard #: 22VAC40-111-690-D Description: Based on observation, the diapering surface was absorbent. Evidence: The changing pads in the one year old classroom had a tears in the cover making it an absorbent surface. Plan of Correction: The provider stated that she would replace the diapering pad.
