Medication log model form can be downloaded from the VA DSS web site, the household member will need fingerprint letter as soon as he returns to Virginia and the substitute time sheet should be completed while the provider is not providing care due to surgery.
An unannounced monitoring visit was conducted today from 11:00-12:25. Nine children were present (24 points) with two caregivers which is within ratios for supervision. Observed children engaged in watching an educational tv show, hand washing, preparation for lunch (provided by the parent), transition to the rest room and preparation for rest time. Emergency preparation is current for trainings, drills and supplies. Two assistant records reviewed as well as two children's records. Day care space has 3 rooms. a kitchen and rest room area and is well equipped. Two caregivers have Medication Administration Training (MAT) certification and one emergency medication was on-site during the inspection. Authorization form and storage were in compliance. Standard #: 22VAC40-111-710 Description: Based on medication review, the medication log is incomplete. Evidence: Medication log documenting the medication given to Child #1 is missing any medication errors and any adverse reaction. Plan of Correction: Downloaded model form and will use this to document when medication is given to a child.
Standard #: 22VAC40-111-180-A Description: Based on record review, the most current report of tuberculosis screening form has expired for 3 caregivers. Evidence: 3 caregivers have TB screening reports that are older than 2 years. Plan of Correction: All 3 caregivers will get updated TB screening forms and will place in personnel record.
