An unannounced renewal study was conducted from 12:00 pm - 1:00pm. There were 7 children in care with the provider ages 23 months, 23 months, 3 years, 4 years, 4 years, 4 years, and 5 years for a total of 15 points. The children were observed participating in free play activities. Lunch served today is peanut butter and jelly sandwich, green beans, watermelon, chips, and milk. Seven children's records were reviewed. The provider and one household member records were reviewed and in compliance. The most recent emergency escape drill was conducted 10/02/2018 and the most recent shelter in place drill was conducted 02/05/2018. There are reportedly no medications being administered to children in the family day home at this time. Description: Based on review of 7 children's records it was determined that not all children's records contained documentation of immunizations for children by the first day of enrollment. Evidence: Child A with a start date of 09/24/2018 did not have documentation of immunization on file.

Plan of Correction: The provider will request updated immunizations from the parents of Child A.

Description: Based on review of CPR training it was determined that the provider did not hold current certification in CPR. Evidence: The most recent CPR certification for the provider was dated 04/02/2016.
