54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted on October 2, 2018 during the hours of approximately 10:30 a.m. and 1:45 p.m. Twelve (12) children were enrolled and eleven children were in the care of the provider and one assistant during the on site inspection. Two resident children were also present in the home (total of 35 points). Five children's records, four caregivers' records and one household member's record were reviewed and/or updated. One child's medication, authorization form and administration record were reviewed. Facility documents were reviewed, safety equipment was observed (first aid kit, flashlight, radio and fire extinguisher) and posted information was viewed (license, licensing inspection documentation, daily schedule, weekly menu, emergency telephone numbers and emergency evacuation diagram/plan). Program observation was completed. Activities observed on this date included: indoor play (toys, books/stories); outdoor play; snack (yogurt and granola bars); diapering, restroom and handwashing procedures; naptime and infant care. Adult/child interactions, behavior guidance and daily routines/transitions were also observed. The children followed the daily routine and transitions well. Previously cited violations were reviewed for compliance (inspection date - 5/21/2018); one repeat violation was cited. Refer to the violation notice for the exceptions to compliance (background clearance, adult/child ratio and medication procedures). Standard #: 22VAC40-111-220-A Description: Based on observation of one child's prescription medication on October 2, 2018 and discussion with caregiver #1, the program failed to obtain the medication prescription label. Evidence: 1. Review of child F's prescription medication, authorization form and medication log, on this date, determined that the medication prescription label had not been obtained. The medication had been administered in August 2018. 2. Caregiver #1 confirmed the lack of the prescription label. Plan of Correction: The child's parent will be contacted and the prescription label will be obtained.
Standard #: 22VAC40-111-570-A Description: Based on observation and discussion with the caregivers on October 2, 2018, the point system exceeded 16 points per caregiver. Caregiver #1 failed to ensure an additional caregiver was present, as needed. Evidence: 1. On this date, eleven children were in the care of two caregivers (31 points). One three-year-old child, three two-year-old children, five toddlers (ages 16 months to two years) and two infants (ages six months and ten months) were present. Additionally, two resident children, ages two years and three years , were present in the home (4 points). 2. Caregiver #1 and caregiver #2 confirmed the number of children in care. Caregiver #1 verified the number of points (35). Plan of Correction: This situation resulted from an error in calculation. There are usually three caregivers present with the children, as needed. In the future, each caregiver will not exceed 16 points.
Standard #: 63.2-1720.1-B-2 Description: Based on review and/or update of five caregiver and household member's records on October 2, 2018, the facility failed to obtain fingerprinting results for one caregiver by September 30, 2018. 1. Review of caregiver #4's record, on this date, determined that fingerprinting results were not on file (employment date - 6/12/2017). 2. Caregiver #1 confirmed the lack of fingerprinting results in the file. Plan of Correction: The caregiver needed to cancel two previously scheduled appointments and completed the fingerprinting process today. In the future, fingerprinting results will be obtained prior to employment for all newly hired staff.
