Standard #: 22VAC40-111-210-A
Description: Based on the review of two adults records, the provider failed to repeat first aid training before the expiration date of the card. Evidence: 1. The first aid card expired on 8/1/18.

Plan of Correction: The provider is enrolled in a first aid class for tomorrow evening.
