An unannounced monitoring inspection was completed on 10//092018. There were three children in care with the licensee. The point total for the three children was six. Five children's records were reviewed, the provider's record was reviewed, assistant caregiver's and the household member's record was reviewed. No medications at the family day home. The inspector arrived at the provider's home at 9:10 am and departed at 12:00 pm. The children were observed during am snack, free time, during a music and movement activity, story time, and during a diaper change. Discussion with licensee about: possibility of taking infants, finger prints.
Standard #: 22VAC40-111-90-A Description: Based on review of five children's records, the licensee failed to ensure that all immunizations were completed every six months for children under the age of two years. Evidence: 1. Child # 1 most recent dated immunization record was completed on 10/24/2016. The date of birth for child # 1 is 4/22/2016. Plan of Correction: The licensee will obtain the update immunization records ASAP.
