Technical Assistance:
Licensing inspector suggested that provider review all children's files to make sure all required information is available.

Comments:
An unannounced focused monitoring inspection was conducted today from approximately 12:50pm-2pm. There were 0 children in care. A sample of 3 children?s records, 1 caregiver's record and 3 household member's records were reviewed. Children were in school during inspection. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-60-B
Description: Based on records reviewed, children's records were incomplete. Evidence: 3 of 3 records reviewed were missing required information. 1) Children A, B and C were missing the address of 1 of 2 required emergency contacts.

Plan of Correction: I will fix the files today.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the provider did not post the most recent violation notice. Evidence: The violation notice posted was for the subsidy inspection. The violation notice for the initial inspection was in a folder.

Plan of Correction: Corrected during inspection.
