Technical Assistance:
Discussed with the provider topical skin products and the form to use.

Comments:
An unannounced monitoring visit was conducted today between the hours of 10:10am through 11:45am with the provider on site. There were 4 children (11 points) with the provider. A sample of 4 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play and snack time. The home is clean, spacious and organized with an abundant supply of age appropriate toys. There were no medication on site today. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on review, 1 of 5 children's records are incomplete. Evidence: 1. Child A's file was missing the medical insurance policy number and the child's immunization record.

Plan of Correction: I will ask the parent for the information today.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation that the child has been adequately immunized before attending the family day home. Evidence: 1. Child A began with the provider on 10/01/2018 and the provider did not have the child's immunization record in the file.

Plan of Correction: I will ask the parent for the immunization record today.

Standard #: 63.2-1720.1-B-2
Description: Based on interview, the provider stated that they had not submitted for fingerprinting. Evidence: 1. The provider and Household Member A had not submitted for fingerprinting by September 30, 2018 and stated she didn't realize it needed to be completed yet.

Plan of Correction: I will set up an appointment to get the fingerprints done.
