Comments:
An unannounced monitoring inspection was conducted today from 9:45am to 10:55am. At the time of inspectors' arrival there were 3 preschool aged children in care totalling 7 points with the provider. Children were observed participating interacting with the provider, handwashing, freeplay, and transitioning between activities. A sample of 5 children's records and 3 provider/household member/substitute provider records were reviewed. Provider reported that no medications are administered at this time and there have been no injuries. Areas of noncompliance are identified on the violation notice and were discussed with the provider during the exit interview at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review and interview it was determined the provider did not ensure that each child's record contained all of the required components. Evidence: 1. The record for child#1(enrolled 9/6/16) did not contain the complete address for one of the two required emergency contacts. 2. The record for child#3(enrolled 12/21/17) did not contain the complete address for one of the two required emergency contacts and the child's medical insurance policy number. 3. The provider confirmed this information was not available during this inspection.

Plan of Correction: Provider will request this information from the parents.

Standard #: 22VAC40-111-90-C
Description: Based on record review and interview it was determined the provider did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The record for child #1 did not contain documentation of current immunizations. The documentation available was dated 2/29/16. 2. The record for child #4 did not contain documentation of current immunizations. The documentation available was dated 11/10/17. 3. The record for child #5 did not contain documentation of current immunizations. The documentation available was dated 10/21/16. 4. The provider confirmed that this documentation was not available during this inspection.

Plan of Correction: The provider will request updated immunization documentation from the parents.
