Technical Assistance:
Provider is required to maintain up to date records on staff and household members which also includes Background Clearances. Any listed caregiver is required to obtain annual training.

An unannounced inspection was conducted from 9 AM- 1045 AM. There were 5 children ( toddler- 4 years) with the Provider and an assistant for a total of 11 points. There was confusion over retaining the finger print registry paperwork and the provider was given a checklist over view to ensure she understands how records are suppose to be maintained. The entire house was walked through but the children receive care on the lower level. Please call with questions 703-479-4679

Description: Based on record review and interviews, the Provider did not conduct monthly evacuation drills monthly. Evidence: 1. Drill record shows last evacuation drill was conducted in July, 2018. Provider said she got behind.

Standard #: 22VAC40-111-60-B
Description: Based on review of 4 children's records. the Provider did not maintain complete files. Evidence: 1. Child #1 is missing one emergency contact's address and missing the child's start date. 2. Child #2 does not have emergency contacts that can be contacted in the event the parent cannot be contacted as it is the Provider and her assistant listed as the emergency contacts. The child's physical is not listed and immunization record is not included.

Standard #: 22VAC40-111-70-B
Description: Based on record review of 4 children's records, the Provider did not obtain the parent's written acknowledgment of receipt of the information that the Provider is not currently MAT certified and not administering medication. Evidence: 1. Parent's acknowledgment form states the Provider may administer medication with the parent's written permission. 2. Provider's MAT certification expired on October 10, 2018. 3. Provider stated she will be taking the class again in December, 2018.

Description: Based on interview, the Provider does not have documentation to verify the wood burning stove was inspected in the last year. Evidence: 1. Provider said she didn't have an inspection summary for the fire place. 2. The last inspection form was from the winter of 2017.
