An unannounced monitoring inspection was completed on 11/8/18. There were two children present with the provider supervising. The point total was six. Three children's records and the provider's record were reviewed. The provider does not administer medications. The inspector arrived at the provider's home at 9:00 am and departed at 12:00 noon. There was discussion with the provider about the following: documentation the source of proof of identity based on those documents that are approved for this purpose, sleeping arrangements of the toddler for overnight, hand washing requirements, requirements for annual training (2017 documentation was available), the upcoming renewal inspection and submitting the renewal application 30 days prior, posting of confidential information.

Description: Based on observation, the provider failed to ensure that children were not allowed to drink while walking. Evidence: 1. A two year old child was observed walking from an adjacent room from the inspector's location where the child was eating snack into the kitchen and then into the room where the inspector was sitting while drinking from a sippie cup. The child walked approximately 15 feet total while drinking from a sippie cup.

Plan of Correction: The provider will not allow the children to drink from a sippie cup while walking around.

Description: Based on interview, the provider failed to ensure that there was a written record of attendance for the date of the inspection. Evidence: 1. The provider was asked at 11:20 am for the written attendance for the day. The provider stated that she did not have it completed.

Plan of Correction: The provider will complete the attendance.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider failed to ensure that two of three children's records contained documentation of complete information that is required by the Standards. Evidence: 1.The record of child #1 was missing documentation of the name and policy number of the child's medical insurance. 2. The record of child #3 was missing documentation of the policy number of the child's medical insurance.

Plan of Correction: The provider will have this information documented.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider failed to ensure that documentation of immunizations were obtained before the child attended the family day home. Evidence: 1. Child #1 had a documented first date of attendance was 9/13/18. The date that the immunization record had a print date on the immunization record as 10/5/18 and was signed by the physician/physician's designee on 10/5/18.

Plan of Correction: The provider will make sure that immunization records are obtained before the child attends in the future.

Standard #: 22VAC40-111-350-E
Description: Based on observation and measurement, the provider failed to ensure that the hot water at taps was maintained within a range of 105 to 120 degrees Fahrenheit. Evidence: 1. The water in the bathroom used by the children was observed to be hot. The inspector measured the water temperature with a digital thermometer. The temperature of the hot water measured 123.8 degrees Fahrenheit. 2. The provider verified the temperature that was showing on the thermometer.

Plan of Correction: The provider stated that she would call someone to turn the water temperature down.

Standard #: 22VAC40-111-540-A
Description: Based on observation, the provider failed to ensure that when an infant carrier seat was used, the safety strap was being used and securely fastened. Evidence: 1. At arrival of the inspector, there was a three month old infant observed sleeping in an infant seat. The safety strap was not fastened. 2. At 10:26, the three month old infant was observed being placed by the provider into an infant rocker seat. The infant safety strap was not fastened when the infant was placed in the rocker seat.

Plan of Correction: The provider corrected this during the inspection. The safety straps were fastened when the inspector mentioned the lack of use to the provider.
