An unannounced renewal inspection was conducted on November 9, 2018, between the hours of approximately 10:30 a.m. and 3:00 p.m. Sixteen (16) children were enrolled and eight children were in the care of the provider and one assistant during the on site inspection. Five children's records, two caregivers' records and one household member's record were reviewed and/or updated. No medications were administered, as is currently the policy at this family day home. Facility documents, safety equipment/supplies and posted information were observed/reviewed. Program observation was completed. Activities observed on this date included: indoor play - toys and television (children's programming); diapering and handwashing procedures; lunch; naptime and infant care. Adult/child interactions, behavior guidance and daily routines/transitions were also observed. The children knew and followed the daily routine and transitions well. Previously cited violations (inspection date - 4/02/2018) were reviewed for compliance; no repeat violations were cited. One exception to standards compliance was noted on the accompanying Violation Notice. Thank you for your time and assistance. Standard #: 22VAC40-111-460-C Description: Based on review of facility records and discussion with caregiver #1 on November 9, 2018, the facility failed to obtain a current rabies vaccination for one family cat. Evidence: 1. Review of documents, on this date, determined that the rabies vaccination for the family cat expired on 6/12/2018. 2. Caregiver #1 confirmed that an updated rabies vaccination had not been obtained. Plan of Correction: An updated rabies vaccination will be obtained for the cat.
