An unannounced renewal inspection was completed on November 1, 2018 from approximately 5:00p.m. to 7:00p.m. One child, five years old, was in care, for a total of one point. The child was observed interacting with the provider during free play and pick up times. Children?s toys, learning materials, equipment, bathroom, outside play area, van used to transport children and the home?s first aid kit and emergency supplies were inspected. Three children?s records, one caregiver record, four household members? records, the emergency response and preparedness plan, fire drill, shelter in place and lock down logs, daily attendance and required postings were reviewed. Medication is not administered and there were no injury reports to review. The provider was available for interview, inspection and exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. There were two citations for violations of the Standards. See the violation notice on the Department's public web site for violations of the Standards.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver and household member records, the licensee failed to ensure that each caregiver and adult household member obtain a tuberculosis screening every two years for one of one caregiver and two of four adult household members. Evidence: 1. Caregiver #1?s record contained tuberculosis screenings dated May 20, 2016 and May 25, 2018, HHM #1's record contained tuberculosis screenings dated May 12, 2016 and May 25, 2018 and HHM #4's record contained a tuberculosis screening dated July 12, 2018. 2. Caregiver #1 acknowledged these screenings were not obtained every two years and indicated this is due to health insurance restrictions. Caregiver #1 was unable to locate HHM #4's previous tuberculosis screening but acknowledged the subsequent screening was over the two year time frame as well.

Plan of Correction: This cannot be corrected. Licensee will find another alternative, such as checking with the local health department, to ensure that the TB screenings are able to be obtained every two years due to health insurance restrictions.

Standard #: 63.2-1721.1-B-2
Description: Based on review of household member records, the licensee failed to obtain fingerprint results by September 30, 2018 for three of four household members. Evidence: 1. HHM #1's record contained fingerprint results dated October 31, 2018, HHM#3's record contained fingerprint results dated October 30, 2018 and HHM#4's record contained fingerprint results dated October 30, 2018. 2. Caregiver #1 acknowledged fingerprints for these household members were not obtained by the September 30, 2018 required time frame. Caregiver #1 indicated that all three household members were out of town for weeks.

Plan of Correction: This cannot be corrected. All household members have eligible fingerprint results.
