Discussed with the provider: If a family does not have a second local emergency contact, they may use one out of state family member.

An unannounced monitorng inspection was conducted beginning at 8:45 and ending at 9:50. There were 3 daycare children , ages 2, 2 and 9 months . A resident child is also in care, age 2. Children were very outgoing and friendly. Reviewed records for all family members and the provider - complete Reviewed records for the three day care children - see Violation notice. Description: Based on interview with the provider it was determined that emergency drills are not done monthly. Evidence: The provider stated that she has not been doing drills, either escape drills of shelter in place.

Plan of Correction: Emergency evacuation drills will be done every month and shelter in place drills will be done twice a year.

Description: Based on a review of three children's records, it was determined that not all records are complete. Evidence: The record for Child #2 does not contain the child's immunization record and prooof of age and identity, The file for Child #3 has only one emergency contact and does not have the Insurance policy number,

Plan of Correction: The provider will get all required information from the parents.
