An announced initial inspection was conducted today, November 15, 2018 to determine compliance with family day home licensing standards. The inspection was initiated at 10:30 am pm and concluded at 12:00 pm. The inspection was conducted with the provider. The provider plans to provide care to children ages birth thru nine years. A capacity of eight children with no more than five children in care after 6:00 pm is recommend based on child care space and zoning. Care will be provided in one main room with additional space in the dining area. The main room is equipped with a variety of age appropriate equipment and supplies including materials for gross motor, literacy and dramatic play. The kitchen-dining area will be used for meals and table activities. One crib was inspected and meets Federal Safety standards. In the kitchen, a parent board is displayed with information posted to include the emergency preparedness plan, menu, and emergency numbers (the license and inspection notices will be posted here). The provider will provide meals and snacks. The menu was posted. Each child has a hook to keep personal belongings on the back of the closet door. One bathroom will be used by children on the first floor of the home. A walk through was conducted for the entire home during today's inspection. Medications will be administered after MAT training has been completed and obtained. The provider will not provide transportation. The provider has current CPR and first aid training and Phase II training was completed on 10/23/2018. There is one pet in the home . The first aid kit and non-medical emergency supplies were inspected (see violation notice). A conditional (6 month) license will be recommended. An unannounced follow up inspection will be conducted within 60 days to determine compliance with standards not determined at the initial inspection. Description: Based on observation, the provider did not obtain all required supplies for the first aid kit. Evidence: The first aid kit did not contain the required triangular bandages.

Plan of Correction: Per the provider: I will purchase the missing items by the weekend.

Description: Based on interview, the provider did not have a working portable battery-operated weather band radio. Evidence: The provider reported she did not have a portable battery-operated weather band radio available.

Plan of Correction: Per the provider: I will purchase a battery-operated weather band radio by this weekend.
