Thank you for your assistance during this unannounced complaint inspection. This on-site visit was initiated from a complaint that was received by the Valley Licensing Office on October 15, 2018 relating to the allegation of records. The information gathered during this complaint supports the allegation, therefore the complaint is valid. Please complete the plan of correction within five business days. Standard #: 22VAC40-111-30-C* Complaint related: Yes Description: Based on review of documentation, text messages and interviews, the provider did not demonstrate good character and reputation when inaccurate information was provided to a child?s parent. Evidence: 1. Interview was conducted with the parent of Child #1. The parent stated she messaged the provider via text, at approximately 3:07 p.m. on October 15, 2018, to ask if her child was still in attendance or had been picked up. 2. On October 15, 2018 at approximately 3:07 p.m., the provider responded to the text message informing the parent that Child #1 "had been picked up right before nap time". On the same date, the provider provided the parent with the name of an individual who allegedly picked up the child when the parent asked who picked up her child. 3. The attendance sign in/out record was reviewed for October 15, 2018. Documentation stated Child #1 was picked up and signed out at 4:33 p.m. 4. Review of text messages exchanged between the parent and provider confirmed the information provided to the parent by the provider. 5. The provider reported that she knew Child #1 had not been picked up when she informed the parent that the child had been picked up and provided the parent with the name of an individual who allegedly picked up Child #1. . Plan of Correction: 1. Amend Hand Book to contain clear expectations in regards to Custody Agreements and my responsibilities required by law 2. Require both parties involved in Custody Agreement to supply a copy of agreement. 3. Amend Hand Book setting guidelines for visitation during hours of operations of daycare. 4. Require sign in and sign out of visitors, date and time.
