63.2 Licensure and Registration Procedures

An unannounced monitoring inspection was conducted today. There are 3 children (10 points) present with 2 caregivers. 6 adult and 3 children's records were reviewed. See violation notice for missing information. Children were observed playing with a variety of toys and equipment today. Emergency escape drills (no children in care in May or June), first aid supplies, emergency supplies and the fridge temperature are all in compliance. I will update the DSS web site to accurately reflect your phone number. Description: Based upon direct observation and provider statement, the menu is not posted.

Plan of Correction: Iwill use the USDA form to post the menu.

Standard #: 22VAC40-111-960-E
Description: Based on direct observation, bottles are not labeled and dated. Evidence: 2 of 2 infant's bottle are not labeled or dated.

Plan of Correction: I will label and date the bottles myself.

Standard #: 22VAC40-111-70-A
Description: Based upon a review of records, children's records do not contain acknowledgment of all required policies. Evidence: 2 of 3 records reviewed did not contain documentation of liability insurance declarations. (Child 1,2)

Plan of Correction: I will have the parent complete the form and place it on file.

Standard #: 22VAC40-111-320-A
Description: Based upon direct observation, small appliances were not unplugged while not in use. Evidence: A hair dryer, toaster oven and coffee machine were all plugged in, in the home area.

Plan of Correction: I will remind everyone to unplug things when they are done using them

Standard #: 63.2-1721.1-B-2
Description: Based upon a review of records, one household member's record is incomplete. Evidence: Household Member #1 does not have documentation of a completed fingerprint search prior to September 30, 2018.

Plan of Correction: They did the fingerprinting, but had to repeat it. I did not receive the results. I will contact OBI to follow up.
