Discussed with provider health and physical forms, maintaining complete records for children, infant feeding procedures, MAT training.
An unannounced monitoring inspection was conducted from 10:30am through 12:45pm. There were 11 children (35 points) with 3 caregivers present. A total of 6 children's files and 6 staff/household member's files were reviewed. The children were engaged in the following: toy play, games, coloring activties, preparing for lunch and nap time. Emergency drill records and medications were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-730-D Description: Based on observation, a non-prescribed medication was not stored in a locked place making it accessible to the children. Evidence: 1. An over the counter gas relief medication was stored in the lowest shelf placed next to the diaper changing table. 2. The shelf was not locked making the medication accessible to the children. Plan of Correction: The medication was removed during the inspection.
Standard #: 22VAC40-111-960-B Description: Based on observation, infants who cannot hold their own bottles were not picked up and held for bottle feeding. Bottles were propped. Evidence: 1. Child #1, age 5 months, was observed in a swing with a milk bottle propped using a soft sheet. 2. It was observed that Child # 1 was not able to hold his milk bottle on his own. Plan of Correction: The child was removed from the swing and a caregiver held the child to feed the bottle.
Standard #: 22VAC40-111-100-A Description: Based on records review, the provider did not obtain documentation of a physical examination for children in care within 30 days of first day of attendance. Evidence: 1. Child # 1's first day of attendance was 9/4/18. The provider did not obtain documentation of a physical examination by a physician. 2. Child # 2 and Child # 3's first day of attendance was 10/15/18. The provider did not obtain documentation of a physical examination by a physician for both children. Plan of Correction: I will ask the parents to provide me with a copy of the physical.
Standard #: 22VAC40-111-220-A Description: Based on observation and staff interview, caregivers did not have the required medication administration training required to administer medication. Evidence: 1. Child # 3's enrollment form indicated that Child # 3 had a diagnosed food allergy. The form indicated that in case of an allergic reaction the child must be given an Epipen. 2. When asked if there was an Epipen was available on site, Caregiver (CG) # 1 stated that there was and the Epipen was brought out for inspection. 3. CG # 2 stated that even though none of the 3 CG's have the medication administration training (MAT), she took it in case the child would need the medication. CG # 2 stated that in case of an emergency she would ask her daughter to administer the medication. CG's daughter is not part of the family day home's staff. Plan of Correction: The parent will be informed, the medication will be returned and we will consider getting the MAT training.
Standard #: 22VAC40-111-280 Description: Based on observation, poisonous substances were not stored in a space that was inaccessible to children. Evidence: A bottle of air freshener and a spray bottle of disinfectant spray was found in an unlocked shelf placed next to the diaper changing area. The shelf was not locked and within reach of the children. Plan of Correction: The bottles were removed and placed on a higher shelf.
