Technical Assistance:
The provider did not have procedures for the continuity of operations to ensure that essential functions are maintain during an emergency included in the emergency preparedness plan. It was discussed with the provider.

Comments:
A Subsidy Health and Safety inspection was conducted on 11/26/18 from 10:30am to 12:45pm to review supplemental health and safety requirements. The children were observed playing with toys, coloring, getting bottle fed, napping and preparing for lunch. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on records review, a written care plan was not obtained for a child in care with a diagnosed food allergy. Evidence: 1. Child # 1's enrollment form indicated that he is allergic to milk and would need an emergency allergy medication in case of an allergic reaction. 2. The provider had not obtained a written action plan to include instructions from the physician in the event there was an allergic reaction.

Plan of Correction: I will obtain an action plan written by a physician from the parent.
