Description: Based on observation and record review, the provider did not ensure all topical skin products had a written authorization. Evidence: 10 creams were found without a written authorization from guardians.

Description: Based on observation, the first aid kit is not complete. Evidence: The first aid kit is missing two triangular bandages.

Plan of Correction: We will buy two triangular bandages.

Description: Based on record review, the provider did not have a written emergency preparedness and response plan. Evidence: The provider could not present the written emergency preparedness and response plan for the family day home.

Plan of Correction: We have a plan but cannot find it. I will fill up a new plan.

Description: Based on observation, the provider did not ensure all required items were in the bathroom. Evidence: There were no paper towels in the bathroom at the time of inspection.

Plan of Correction: The provider placed paper towels in the bathroom once notified there was none at the time.

Description: Based on observation, children sitting in a high chair were not securely fastened using the safety strap. Evidence: At 11:00am, two children were observed in high chairs without being securely fastened using the safety straps on the high chairs.

Plan of Correction: At 11:05am, the inspector discussed with the assistant caregiver that all children must be securely fastened in high chairs. Children were immediately strapped in safely.

Description: Based on record review, the provider did not have the findings from the most recent inspection of the family day home posted. Evidence: The inspection findings for the inspection dates 3/26/2018 were not posted and could not be presented at the time of inspection.

Plan of Correction: I will make sure to keep the violation notice posted.
