54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring inspection was conducted today from approximately 10:15am-12:40pm. There were 5 children (18 points) in the direct care and supervision of 2 caregivers. A sample of 3 childrens' records, 2 caregivers' records and 1 household members' record was reviewed. The children were observed eating lunch, playing, singing, drawing, and preparing for nap. The provider is in the process of applying for a capacity of 12 and moving her child care to the basement. Zero new caregivers have been hired since the last inspection. Description: Based on observation, the radio and flashlight were not battery operated. Evidence: The radio and flashlight required a wall charger to be operated.

Plan of Correction: I will order it online now and get it within 3 days.

Description: Based on record review, the assistants record was incomplete. Evidence: The file of Assistant A was missing an emergency contact, and age verification.

Plan of Correction: I will complete it right now.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were incomplete. Evidence: 1 of 3 records reviewed were missing required information. 1) Child A was missing the fathers employer and employer phone number.

Plan of Correction: I will have the parent fix it today

Description: Based on record review, the provider did not orient the assistant by the end of the first week. Evidence: Assistant A did not have proof of orientation in the file.

Description: Based on interview with staff, the wood burning fireplace were not inspected annually. Evidence: The an annual fire inspection was not conducted on the wood burning fireplace. The provider stated that she does not use the fireplace and the fireplace has not had an inspection. The last fire inspection was conducted 06/18/2013
