Technical Assistance:
As discussed there is a new Sworn Statement forrm online which includes out of state residency question.

An unannounced monitoring inspection was conducted this date. A total of 11 children (28pts) in care with provider and 1 assistant. One child is related to the The children were observed completing wooden puzzles at the table top, playing with each other with toys and books. Children observed transitioning to gross motor skills which included clapping and dancing to music and singing along. Hand washing prior to lunch was observed. Crib area inspected. Only two in care are sleeping in cribs. The home provides a great deal of activities, toys and equipment for the children. All of which are located within reach of the children to encourage independent selections. A total of 7 children's records, all staff and household records were reviewed. No medication on site for the children and no identified food allergies. This inspection began at 10:15am and ended at 12:20pm. Thank you for your cooperation today. Please email me at:

Description: Based on review of 7 children's records, documentation of immunizations were not obtained prior to the child's first day. Evidence: Child #1 began care on 7/23/2018. Immunization documentation is dated 8/31/18.

Plan of Correction: I will make sure the parent provides this documentation prior to the child's first day.
