Comments:
This unannounced renewal inspection was conducted today from 9:40am-11:45am. 3 children (7 points) with provider was on site today. Children were observed engaging in toy play and dancing and singing along with a television program. 3 children's records, provider record, and all household members records were reviewed. Observation of all 3 levels of the home was completed, some violations found regarding the physical plant. A nice transition was made coming from the middle level to the lower level, the programming area. Provider was singing songs with the children in care and positive interaction was observed. No medications or food allergies identified. Current renovation on the lower level is in progress, which has not impacted care of the children. Thank you for your cooperation today. Standard #: 22VAC40-111-800-B
Description: Based on review of the provider's emergency plan, the plan was not reviewed annually. Evidence: 1. The emergency evacuation plan was last updated May 2017.

Plan of Correction: Provider will update the emergency evacuation plan. There are no changes to the plan.

Standard #: 22VAC40-111-60-A
Description: Based on record review, the provider did not maintain an up-to-date record at the family day home for each enrolled child. Evidence: 1. No record was found for Child #3.

Plan of Correction: Provider will obtain the record back from this parent.

Standard #: 22VAC40-111-60-B
Description: Based on child record review, one record was incomplete. Evidence: 1. Child #1's record did not have documentation of proof of identity.

Plan of Correction: Provider will obtain a copy of the proof of identity from parent and document.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the home, one area was found not maintained in a clean and safe condition. Evidence: 1. The area outside of the perimeter doors in the basement were covered with 12" of leaves, prohibiting proper drainage and a hazard in the event that children need to evacuate.

Plan of Correction: Provider will rake out the leaves.

Standard #: 22VAC40-111-320-B
Description: Based on observation of the home, not all electrical outlets were found with child-resistant protective covers. Evidence: 1. Multiple child-resistant protective covers were missing in the kitchen and living room.

Plan of Correction: Provider will ensure that all electrical outlets are covered.
