An unannounced renewal inspection was completed on 12/04/2018. Present during the inspection was 1 child, aged 20 months, totaling 3 points, in the care of one provider. The inspector reviewed 1 child's records, one provider records, one household member record, and two pet records during the inspection. The provider stated that medication is not administered. The child was observed having a snack and in free choice play. The inspector reviewed the medication policy, the updated emergency preparedness plans, the emergency supplies, and the required information for parents during the inspection today. The licensing inspector discussed the new model forms available to providers and the changes to the subsidy requirements with the provider. Standard #: 22VAC40-111-770 Description: Based on observations and discussion, the provider failed to maintain a working battery-operated flashlight and a working portable battery-operated weather band radio that is available to caregivers at all times. Evidence: The battery-operated flashlight/weather band radio did not work. Plan of Correction: The provider will purchase a new one.
Standard #: 22VAC40-111-60-B Description: Based on record review and discussion, the provider failed to ensure that the record for each child shall contain all of the elements required by the standards. Evidence: The record for Child #1 did not contain the following information: proof of the child's age and identity and the names and addresses of previously attended child day care and the written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. Plan of Correction: The provider will get the missing information from the parent.
