Comments:
A mandated Renewal inspection was conducted today from 10:00-11:30. There was 1 provider and 1 child present (3 points). Reviewed 3 resident files, 2 children's files, emergency drills, first aid kit, menu, attendance, cribs, and physical plant. The child was observed reading a book, playing with a ball, and playing with a musical manipulative. Violations of the Standards are recorded on the Violation Notice. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on review, the first aid kit was incomplete. Evidence: The first aid kit was missing 2 triangle bandages and a working digital thermometer.

Plan of Correction: I will get them

Standard #: 22VAC40-111-330-A
Description: Based on interview with the Provider, a land line telephone was not available during the family day home's hours of operations. Evidence: Provider stated she did not own a land line phone, only a fax and a cell phone.

Plan of Correction: I will purchase the phone
