A mandated Renewal inspection was conducted today from 3:45 pm to 5:25 pm. There were 8 children and 3 caregivers present (18 points). Children were observed having snack, playing with puzzles, card games, musical manipulatives, plastic animals, and tummy time. Reviewed 2 staff records, 4 household members records, 2 children's files, attendance, menu, emergency evacuation drills, first aid supplies, cribs and physical plant. Description: Based on observation, first aid kit was incomplete. Evidence: First aid kit is missing 1 triangle bandage.

Description: Based on record review, documentation that two or more references as to character and reputation as well as competency were not checked before employment. Evidence: Staff#1, start date 9/6/16, was missing documentation for 1 reference.

Description: Based on record review, parent's written acknowledgement of receipt of Parent Information was not obtained. Evidence: Child#1, start date 7/9/18, did not have documentation of Information to Parent Statement being received by the Parent.
