Comments:
An unannounced renewal inspection was conducted today, December 4, 2018 to determine the providers compliance with family day home standards. The inspection was initiated at 1:00 pm and concluded at 3:15 pm. Upon the inspectors arrival five children were in care with the provider for a value of twelve points. One infant was observed in an exersaucer with the provider sitting next to him. The other four children were napping. The provider conversed and played with the infant. Later after nap, children listened to a story about different trucks. The provider asked questions and the children pointed out different items in the story. The children danced to children's music and showed the inspector different dance moves. Today's lunch consisted of chicken breast, broccoli, mashed potatoes, wheat bread and milk. The family day home has age appropriate materials. The provider is working with the infant Toddler program to create an additional area for the children to use within the home. The following required information was posted: the License, most recent inspection notice, emergency numbers, daily schedule, food allergies/dietary restrictions/food sensitivities, and menu. The first aid kit and non-medical emergency supplies were inspected and found complete with extra items available. The inspector interacted with the caregiver and children in care. Two caregiver records were reviewed along with five children's records. The caregivers have current CPR and first aid training. Medications are not being administered. The provider does not provide transportation. The most recent evacuation drill was documented on 1121/2018. The two shelter-in-place drill for 2018 were completed on 06/18/2018 and 10/18/2018. A lock down drill was conducted on 06/15/2018. The attendance record was reviewed. The emergency preparedness plan was current. Children do not use the deck on the rear of the house. Violations cited during the licensing period were checked for corrections. There were no repeat violation cited during today's inspection. Standard #: 22VAC40-111-100-A
Description: Based on review of children's records, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child #1, enrolled 09/06/2018, did not have documentation of a physical examination on file exceeding the 30 days allowance. 2. Child #2, enrolled 09/06/2018, did not have documentation of a physical examination on file exceeding the 30 days allowance.

Plan of Correction: Per the provider: I will call the parent now and tell her i need a copy of the physical. Per the provider, the parent reported she would fax the one physical and that the child had an appointment tomorrow and she would get the physical for the records.
