An unannounced monitoring inspection was conducted today from approximately 12:45-2:45pm. There were 9 children (22 points) in the direct care and supervision of 4 caregivers. A sample of 5 children?s records, 4 caregiver's records, and 1 household member's record was reviewed. The children were observed during nap time. 1 new caregiver has been hired since the last inspection. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-800-B Description: Based on observation, the emergency plan was not reviewed annually. Evidence: The Emergency Plan was last reviewed 2016. Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-111-120-B Description: Based on record review, assistant's records were missing required information. Evidence: Assistant A was missing address, age verification, and emergency contact. Plan of Correction: I will complete it right now.

Evidence: 2 of 5 children's records reviewed were missing required information. 1) Child A and B were missing father's employer phone number. Plan of Correction: I will ask them to update files today.
Standard #: 22VAC40-111-530-A Description: Based on observation, children other than infants sleeping in a crib did not have a top cover. Evidence: Child C and D were sleeping in a crib without a top cover. Plan of Correction: We will put a top cover on them, my previous inspector told me to remove everything from the crib.
