Standard #: 22VAC40-111-90-A
Description: Based on a review of five records for children, and an interview with the provider, the provider failed to obtain documentation that a child was immunized before initial attendance. Evidence: Child 2 began attending on November 5, 2018 and the only immunization report was completed on November 20, 2018.

Plan of Correction: The provider will tell parents of new children that the immunization report must be provided with the enrollment forms and before children start receiving care.

Standard #: 22VAC40-111-90-C
Description: Based on a review of five records for children, and an interview with the provider, the provider failed to obtain an updated immunization record at least once every six months for a child under age two years. Evidence: The most recent immunization report for child 1, who is under age one year, was obtained in May 2018. The updated immunization record was due by November 30, 2018.

Plan of Correction: The provider immediately requested the record and found out the parent has it. The updated record will be obtained for the file. The provider will continue to do a regular record review to determine record needs and will text a reminder to parents and remind them of needed information.
