63.2 Licensure and Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on 12/11/2018 from 1:50pm - 2:40pm. There were no children in care and the provider stated that the family day home had been temporarily closed and would resume operation in January of 2018. As a result standards in the areas of children's records, care of children, hand washing, diapering, medication, and nutrition were not determined. The inspector reviewed the child care area. During the tour of the home physical plant safety was discussed. The inspector reviewed the outdoor play area. Records were reviewed for the provider and one household member. The violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation and interview, the provider failed to ensure that the first aid kit for the family day home contained all of the required elements. Evidence: 1. The first aid kit lacked a digital thermometer. a. The provider confirmed that a thermometer was not available.

Plan of Correction: A digital thermometer will be obtained today.

Standard #: 22VAC40-111-770
Description: Based on observation, the provider failed to have a working battery-operated weather band radio and extra batteries available at all times. Evidence: The provider was not able to get the battery operated radio to work during the inspection.

Plan of Correction: Batteries for the radio will be obtained today.

Standard #: 22VAC40-111-830-C
Description: Based on record review and interview, the provider failed to maintain documentation of emergency evacuation and shelter-in-place drills. Evidence: 1. The emergency log for 2018 was not available for review. 2. The provider confirmed that the 2018 emergency practice log for the home could not be located.

Plan of Correction: Fire drills and shelter drills have been conducted however, the practice log has been misplaced. The log will be located or recreated.

Standard #: 22VAC40-111-140-B-3
Description: Based on record review and interview, the provider failed to maintain current certification in CPR as appropriate to the age of the children in care. Evidence: 1. The CPR certification for the provider expired on 06/30/2018. 2. The provider confirmed that her CPR certification had expired.

Plan of Correction: A CPR class is scheduled for Sunday, December 16, 2018.

Standard #: 22VAC40-111-140-B-4
Description: Based on record review and interview, the provider failed to maintain current certification in first aid as appropriate to the age of the children in care. Evidence: 1. The first aid certification for the provider expired on 06/30/2018. 2. The provider confirmed that her first aid certification had expired.

Plan of Correction: A first aid class is scheduled for Sunday, December 16, 2018.

Standard #: 63.2-1721.1-A
Description: Based on record review and interview, the provider failed to ensure that household members have a fingerprint background check. Evidence: 1. There was no fingerprint background check available for household member 1. a. The provider confirmed that household member 1 did not have a fingerprint background check.

Plan of Correction: Household member 1 had the fingerprint background check done on 12/10/2018. The results of this check will be forwarded to the inspection when received.
