Technical Assistance:
Lock-down procedures shall be practiced at least annually.

An unannounced Subsidy Health and Safety Inspection was conducted with the provider in conjunction with the monitor inspections beginning at 9:00 AM and ending at 10:40 AM. There were four children in the care of the provider. The children's ages were 9 months, 11 months, 15 months, and 2 years old for a total of 14 points. The children were observed dancing to music, being read a story, and playing with blocks and instruments. Four children's records were reviewed. Three household records were reviewed and in compliance. There were no medications on site to be reviewed. There were no allergy care plans to be reviewed. The emergency escape drill log was reviewed. The first aid kit, flashlight, and weather band radio were reviewed and in compliance. Description: Based on a review of documentation, the caregiver did not have documentation of completion of Virginia Pre-Service Training for Child Care Staff sponsored by the Department of Social Services. Evidence: There was no documentation of Virginia Pre-Service Training in the provider's record.

Plan of Correction: The provider could not locate the certificate of completion for the Virginia Pre-Service Training during the inspection. She stated that she would look through files of previously completed trainings to locate it and place it on the board or in the record.

Description: Based on a review of documentation, the provider did not conducted an annual lockdown drill. Evidence: The provider did not have documentation of a lockdown drill be conducted.

Plan of Correction: The provider stated that she would conduct a lockdown drill and document it.
