An unannounced renewal inspection was conducted with the provider on December 13, 2018 beginning at 9:00 AM and ending at 10:40 AM. There were four children in the care of the provider. The children's ages were 9 months, 11 months, 15 months, and 2 years old for a total of 14 points. The children were observed dancing to music, being read a story, and playing with blocks and instruments. Four children's records were reviewed. Three household records were reviewed and in compliance. There were no medications on site to be reviewed. There were no allergy care plans to be reviewed. The emergency escape drill log was reviewed. The first aid kit, flashlight, and weather band radio were reviewed and in compliance. Standard #: 22VAC40-111-60-B Description: Based on a review of documentation, not all of the children's records were complete. Evidence: Child A's record was missing the telephone numbers for the parents place of employment and the medical insurance policy number. Plan of Correction: The provider stated that she would obtain the missing information from the parents.
Standard #: 22VAC40-111-520-C Description: Based on observation and interview with staff, the vendor could not provide documentation that all cribs met current Consumer Product Safety Commission (CPSC) Standards. Evidence: A steel folding crib had a manufacture label dated 10/10/2005 and did not have a certification that it met the current CPSC Standards. Plan of Correction: The provider stated that she would contact the manufacter to obtain this information. If the provider was unable to obtain certification that the crib met current CPSC Standard, the crib would be removed.
