Discussed play pen usage and cribs and attendance records with the provider.
An unannounced renewal visit was conducted today between the hours of 8:30 am through 10:00am with the provider on site. There were 11 children (18 points) with the provider and assistant. A sample of 4 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, snack time and preparing to go on a field trip. The physical space, emergency supplies, evacuation drills and attendance records were reviewed. Areas of non-compliance are identified in the violation notice.
Standard #: 22VAC40-111-750-A Description: Based on review, the provider did not meet all requirements when using topical products. Evidence: 1. Child A, Child B and Child C had diaper creams labeled with the child's name on it and the provider did not have authorization forms from the parent. Plan of Correction: I will ask the parents to fill out the forms today.
Standard #: 22VAC40-111-320-A Description: Based on observation, small electrical appliances were not unplugged when not being used. Evidence: 1. A toaster was plugged in the kitchen upstairs and was not being used. Plan of Correction: I will unplug after use.
