Technical Assistance:
Consultation provided on the new sworn disclosure forms and the twice a year shelter in place drills do not take the place of monthly escape drill.

An unannounced monitoring inspection was conducted with the provider, who was present with two napping preschoolers ( 2 and 3 years of age) for a total of 4 points. The provider does not administer medication and reported that there have been no injuries or accidents since the last inspection. Seven children and three adult files were reviewed today. Arrival time: 12: 35 pm Departure time: 2: 15 pm

Standard #: 22VAC40-111-830-A
Description: Based on a review of the monthly escape drill log and an interview with the provider, it was determined that the provider is not conducting monthly escape drills. Evidence: There were no escape drills for May, September and November 2018.

Plan of Correction: I will remember to do monthly drills.

Standard #: 22VAC40-111-200-A
Description: Based on a review of three adults files and an interview with the provider, it was determined that Person A was not provided orientation training by the end of their first week of assuming job responsibilities. Person A has been the assistant since March 2018.

Plan of Correction: I will give the training and keep documentation in the file.

Standard #: 63.2-1720.1-B-3
Description: Based on a review of three adult files( household members and assistant) it was determined that one person did not have an out of state child abuse and neglect search request by the end of the 30th day or employment/service. Evidence: Person A indicated that they lived in New York within in the last five years on the sworn disclosure statement dated 3/10/18. The provider has not sent off for this out of state background check.

Plan of Correction: I will send off for the check within the next two weeks.
