63.2 Licensure and Registration Procedures

An unannounced monitoring inspection was completed on 12/28/18. There were two children in care, 20 months and four years, with the provider and assistant supervising. The point total was five. Seven children's records, the provider's record, and the assistant/household member's records were reviewed. The provider does not administer medications. The inspector arrived at the provider's home at 11:45 am and departed at 1:15 pm. The children were observed during nap. There was discussion with the provider about the following: needing to practice and document a second shelter-in-place drill for 2018, allowable variance.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider failed to ensure that all required information was documented in each child's record. Evidence: 1. The record for child #1 was missing the following require information: a work phone number for the mother and the father of the child (both had documented work places outside of the home), the policy number for the child's medical insurance (a medical insurance name was provided). 2. The record for child #7 was missing the work phone number for the father of the child.

Plan of Correction: The provider will have the parents document the missing information.

Standard #: 22VAC40-111-140-B-3
Description: Based on interview, the provider failed to ensure that current certification in first aid and CPR was maintained. Evidence: 1. The provider was asked for documentation of current first aid and CPR certification. The provider could not locate the certification card but stated that the certification expired in October 2018. The provider stated that she had scheduled a class in October that had to be cancelled and a class has not been re-scheduled to renew the certifications.

Plan of Correction: The provider will schedule a class for January or February at the latest.
