Discussed diaper ointments, staff information sheet in the renewal application, and menu for the children's snacks/meals.
Today an unannounced renewal inspection was conducted between 9:00am-10:20am. There were 5 children (14 points) present in the direct care and supervision of 2 caregivers. The children were observed looking at books, playing with toys, and having morning breakfast consisting of milk, fruit, cheese, and crackers. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Thank you for your cooperation. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child #5 (start date 9/4/2018) was missing documentation of a physical examination. Plan of Correction: I will ask the parent for this.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were not complete with all the required information. Evidence: 1. 5 out of 5 children's records were missing the family day home's emergency plan. 2. 5 out of 5 children's records were missing the family day home's medication policy. Plan of Correction: I will give these to the parents to sign.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider did not maintain documentation of a current Report of Tuberculosis Screening for each adult household member. Evidence: 1. Household member #1 had an expired Tuberculosis (TB) screening. The last TB screening on file was dated 12/23/2014. Plan of Correction: The household member will get a TB screening.
