An announced initial inspection was conducted with the provider beginning at 10:00 a.m. and 1:30 p.m. The daycare areas were observed. Childcare areas are well organized. There are ample supplies and equipment for the requested age range of one month through five years. There were no children in care today and not all standards could be reviewed. One provider record, one household member record and four children's records were reviewed and in compliance. First aid supplies, a battery operated radio and flashlight were observed. The provider is not Medication Administration Training (MAT) certified and cannot administer medications to children in care. Description: Based on review of the first aid kit, not all required supplies were available. Evidence: There was no First aid instructional manual.

Plan of Correction: A first aid instructional manual will be obtained and added to the first aid kit.

Standard #: 22VAC40-111-240-A
Description: Based on observation, interview with the provider and review of documentation, not all equipment in the family day home was maintained in safe condition. Evidence: A Pottery Barn brand crib had a manufacture date of 12/2009, An Eastside brand crib had a manufacture date of 6/22/2010 and Crib #4 did not have a manufacturer label or date.

Plan of Correction: The manufacturers of the cribs will be contacted for the required documentation. If the documentation cannot be obtained or verified, the cribs will be removed and new cribs will be obtained.
