The licensing inspector conducted an unannounced monitoring inspection from 9:20am - 12:00pm. There were two children in care for a total of four points with one caregiver. The children were observed playing with toys, watching cartoons and having snacks. One caregiver record, three household member's records and two children's records were reviewed. Violations from the previous inspection were also reviewed and two were repeated. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-111-830-A
Description: Based on review of the documented drills, the family day home failed to practice monthly emergency evacuation drills. Evidence: 1. The inspector reviewed the documented drills. The family day home failed to practice emergency drills for the months of July through December 2018.

Plan of Correction: The provider will practice emergency evacuation drills monthly and do some extra drills to make up for the missing drills.

Standard #: 22VAC40-111-830-B
Description: Based on review of documentation of the shelter-in-place drills, the family day home failed to practice shelter-in-place drills. Evidence: 1. The inspector reviewed the family day home's drill documentation. The family day home failed to practice any shelter-in-place drills for the calendar year 2018.

Plan of Correction: The provider will practice and document at least two shelter-in-place drills this calendar year.

Standard #: 22VAC40-111-90-C
Description: Based on record reviews, the family day home failed to obtain documentation of updates to child's immunizations. Evidence: 1. The inspector reviewed two children's records. One of two records reviewed (child record #1) did not contain updated information for immunizations every six months for child under two.

Plan of Correction: The provider will ask the child's parents again for the child's updated immunization information today.

Standard #: 22VAC40-111-180-A
Description: Based on record reviews, the family day home failed to obtain repeat tuberculosis screenings. Evidence: 1. The inspector reviewed one caregiver's record and three adult records. All four records contained tuberculosis screenings that were over two years old. Caregiver record #1's screening was dated 2/3/16, adult record #1's screening was dated 3/28/16 and adult records #2 and #3's screenings were dated 5/31/16.

Plan of Correction: All adults will have repeat tuberculosis screenings by the end of January.
