Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on January 10, 2019 from 11:30 a.m. through 1:00 p.m. There were three children in the care of the provider for a total of six points. Three children's records were reviewed as well as the record for the provider. No medication is being administered at this time. Areas observed were hand washing/bathroom procedures, lunch preparation, lunch and nap. Interactions between the children and staff were observed as positive and the children seem comfortable with the daily routine and schedule. Standard #: 22VAC40-111-770 Description: Based on observation, the provider failed to have a working battery operated flashlight. Evidence: 1. The provider put new batteries in the flashlight. 2. The flashlight would not work when turned on. Plan of Correction: I will purchase a new flashlight.
