Technical Assistance:
Discussed with the provider modification procedures in order to modify her license to reflect the Family Day Home's (FDH) new address. The provider has submitted a modification form to the Licensing Office and is currently working with the Local Zoning Administration. The provider will obtain a Contact with Local Zoning Administrator form and submit it to the Fairfax Licensing Office prior to the issuance of the modified license. In order to obtain a permit through zoning for a number higher to the "by right" number children established by the County of Fairfax, a special exception application will need to be completed and approved by the County of Fairfax Zoning.

Comments:
An announced monitoring/modification inspection was conducted today from 9:30am-11:00am at the provider's new Family Day Home (FDH). There were a total of 7 children (18 points) with two caregivers. A total of 5 children's records, 1 household member's record, and 1 staff records were reviewed. The children were observed playing freely, being bottle fed, playing with musical instruments, listening to an interactive story, and eating lunch. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-111-800-A
Description: Based on review of the home's emergency plan and interview, it was determined that the provider does not have a current Emergency Preparedness Plan. Evidence: The provider has not updated the home's Emergency Preparedness Plan to reflect the location of her new home and the revised emergency relocation area.

Plan of Correction: We will update the Emergency Preparedness Plan form.

Standard #: 22VAC40-111-70-A
Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before a child's first day of attendance. Evidence: Child#4's record is missing the family day home's Liability Insurance Declaration Form and a complete and current emergency preparedness and response plan.

Plan of Correction: We will update the information and have the parents sign the new forms.

Standard #: 22VAC40-111-320-G
Description: Based on review of documentation and interview, the wood burning fireplace located on the main level of the home has not been inspected by a knowledgeable inspector to verify that the device was properly installed, maintained, and cleaned as needed. Evidence: During the inspection a wood burning fireplace was observed in the family day home; however, there was no documentation of a fireplace inspection.

Plan of Correction: We will have it inspected.
