Description: Based on record review and provider statement, shelter-in-place drills were not practiced a minimum of twice per year. Evidence: No shelter-in-place drills were practiced in the previous year.

Plan of Correction: I will practice a minimum of two shelter-in-place drills per year.

Standard #: 22VAC40-111-60-B
Description: Based on record review, three of five children's records reviewed were incomplete. Evidence: 1. Child 1's record did not contain the father's employment information or the contact information for two required emergency contacts. 2. Child 4's record did not contain the addresses and phone numbers of two emergency contacts. 3. Child 5's record did not contain information for the parents' place of employment.

Plan of Correction: I will ask parents to update the records.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the provider did not provide in writing documentation of whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 63.2-1809.1 of the Code of Virginia. Evidence: Child 2 and 4's record contained a blank and unsigned liability disclosure form.

Plan of Correction: I will have the parent's sign a completed form as acknowledgement.

Standard #: 22VAC40-111-240-A
Description: Based on observation, not all areas of the family day home were maintained in a safe condition. Evidence: An unattended burning candle was observed in the kitchen on the floor above the child care area.

Plan of Correction: I will blow out the candle and will not burn candles while the child care is operating.

Standard #: 22VAC40-111-250-A
Description: Based on observation, strangulation hazards were present in the child care area. Evidence: 1. A looped window cord was observed hanging from a window in the childcare area. 2. An appliance cord was observed draped from a large television on a high shelf before being loosely tucked behind a shelf within reach of children in the child care area.

Plan of Correction: The provider noticed the hazard prior to the exit conference and secured all hazards immediately.
