Comments:
An unannounced monitoring inspection was conducted from 11:00 am - 12:15 pm. There were 12 children in care with the provider and one assistant ages 6 months, 12 months, 15 months, 17 months, 18 months, 19 months, 19 months, 2 years, 2 years, 2 years, 3 years, and 3 years for a total of 34 points. The children were observed preparing for and eating lunch. Lunch is provided from home. Twelve children's records were reviewed and in compliance. The provider and one assistant records were reviewed and in compliance. The most recent emergency escape drill was conducted 01/14/2019 and the most recent shelter in place drill was conducted 01/22/2019. There are reportedly no medications being administered to children in the family day home at this time. The provider is current in CPR, First Aid and MAT (Medicaiton Administration Training) certification. Description: Based on review of 12 children's records and interview with staff it was determined that the provider failed to ensure that caregivers were not caring for children exceeding16 points. Evidence: The provider and one assistant were caring for 12 children ages 6 months, 12 months, 15 months, 17 months, 18 months, 19 months, 19 months, 2 years, 2 years, 2 years, 3 years and 3 years for a total of 34 points.

Plan of Correction: One child was in care that was not expected. The provider and assistant will not care for more than 12 children together exceeding 32 points.
