22VAC40-111-380 - Adequate spacing of the play area was discussed with the provider.
Licensing inspector arrived at 9:30am and departed at 10:55 am to conduct a monitoring non-mandated inspection . At the time of the tour there was 7 children in care with 2 staff (22 points). Children were observed watching TV, dancing in bouncy seats, playing with trucks and coloring. The sample size consisted of 5 children records and 2 provider records. Areas of non-compliance was reviewed with the provider.
Standard #: 22VAC40-111-60-B Description: Based on 4 of 5 records reviewed , provider did not ensure children's records has documentation of required information. Evidence: During the inspection on January 29, 2019 the following records did not have required information: Child #1 did not have second emergency contact. Child #2 did not have a address number . Child #3 did not have second emergency contact with address, city , state and zip code. Child #4 did not have a second emergency contact. Plan of Correction: The provider will ask the parents for a 2nd emergency contact.
Standard #: 22VAC40-111-280 Description: Based on observation, the licensee did not ensure poisonous substances are inaccessible to children. Evidence: During the inspection conducted on January 29, 2019, the following areas there were poisonous substances were accessible: In the kitchen , there was shea butter and Epsom salt. The poisonous substances were labeled with Caution, Warning and Keep out of reach children. Plan of Correction: The the poisonous substances were removed.
Standard #: 22VAC40-111-390-A Description: Based on observation , the licensee did not ensure each child who is two years of age and older have access to an individual location in which to keep clothing, toys, and belongings. Evidence: During the inspection conducted on January 29, 2019, there were coats laying on top of each other on a chair. In the dining room area there were blankets and sheets piled on top of each other on the cots. Plan of Correction: The belongings will be in individual cubbies.
Standard #: 22VAC40-111-600-B Description: Based on observation the licensee did not ensure infants and toddlers spend no more than 30 minutes of consecutive time confining in piece of equipment. The intervening time period between confinements shall be at least one hour. Evidence: During the inspection conducted on January 29, 2019, child #3 and child #4 were in bouncy seats from 9:28am to 10:18am ( 50 minutes). Child # 4 was placed back into the bouncy seat at 10:44. There was not a intervening time period of at least hour between the last time child #4 was taken out of the bouncy seat. Plan of Correction: The provider will keep track of the time the infants are in the bouncy seats.
