Comments:
An unannounced renewal inspection was conducted today from 10:40am-12:15pm. There were 6 children (18 points) supervised by 3 direct care staff. The home, 2 staff records, 4 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed coloring and engaging in free play. Diapering and hand washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-830-B
Description: Based on record review, the provider did not maintain documentation of shelter-in-place procedures practiced twice per year. Evidence: The provider had one documented shelter-in-place drill for 2018.

Plan of Correction: We will be sure to practice two shelter-in-place drills this year.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 2 of 4 children's records did not contain documentation of all required information. Evidence: 1. Child #3's (start date: 10/1/2018) record did not contain documentation of either parent's work telephone number, and medical insurance policy number. 2. Child #6's (start date: 7/30/2018) record did not contain documentation of dad's work telephone number and the medical insurance name and policy number.

Plan of Correction: I will ask parents today and tomorrow for missing information.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diaper surface was not nonabsorbent and washable. Evidence: There were several tears in the diaper changing pad.

Plan of Correction: A new diaper changing pad as already been ordered.
