Technical Assistance:
Discussed with provider: Increase in capacity by 1 for the time being, withthe potential to increase an additional child at the renewal study in August.

An unannounced monitoring inspection was conducted today. There are 7 children present (19 points) with two caregivers. 2 adult and 2 children's records were reviewed. See Violation notice form missing information. Children were observed resting, washing hands, eating snack and being fed. A Subsidy Health and Safety Inspection (SHSI) was also conducted today.

Standard #: 22VAC40-111-940-A
Description: Based on direct observation, the refrigerator temperature was not maintained at 40 degrees. Evidence: The thermometer available for review registered a temperature of 50 degrees.

Plan of Correction: I will lower the temperature on the fridge.

Standard #: 22VAC40-111-60-B
Description: Based on areview of records, one child' record does not contain all required information. Evidence: Child 1's record does not include documentation of having viewed Proof of Birth within 7 days of attendance. (Date of Attendance: 11/8/18)

Plan of Correction: I will ask the parent t provide the information.

Standard #: 22VAC40-111-240-A
Description: Based on direct observation, the home does not have smoke detectors in working order. Evidence: There are 2 "chirping" smoke detectors in the home.

Plan of Correction: I will replace the batteries in all detectors.
