The licensing inspector conducted an unannounced monitoring inspection on 02/06/2019 from 9:15am to 1:00pm. The children were playing with toys upon the inspector?s arrival, and they were later observed playing games, singing and counting. Today, there were three children, totaling 6 points, present with one caregiver. The caregiver was observed interacting with the children. Interviews were held with the caregiver throughout the inspection. Additionally, when appropriate the inspector interacted with children. Lunch consisted of corn, cheese, bread, carrots and milk.
The provider does not administer medication.
The provider does not provide transportation.
The provider does not have a pet.
Three children?s records and two caregivers? records were reviewed during the inspection.

The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.

Standard #: 22VAC40-111-60-B Description: Based on record review, the provider did not ensure that two of three children's records contained confirmation of up-to-date information.
1. Child #1 was enrolled on 09/5/2017 and the record was last updated 8/20/2017. Child #2 was enrolled on 01/6/2016 and the record was last updated 1/4/2017.
2. Child #3 (start date: 9/5/18) did not have documentation of a signed parent acknowledgment disclosing the provider's policy for medication administration.
Plan of Correction: I am going to ask parents to review and update information today. I am going to find the policy for medication administration and get the parent to sign it today.
Standard #: 22VAC40-111-240-A Description: Based on observation, the provider did not ensure that areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition. Evidence: 1. The provider had television and cable box sitting on a shelf. The cords from the equipment were hanging presenting an entanglement hazard for the children and the equipment could possibly fall on a child if pulled. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-111-240-A Description: Based on review, the provider did not ensure that two of two caregiver records contained documentation of a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
The record of caregiver #1 contained documentation for 3 hours of annual training. The record of caregiver #2 contained documentation for 13 hours of annual training.
Plan of Correction: I am going to find and take trainings online.
